Ohio’s Hospice is seeking a Medical Social Worker to provide psychosocial assessment, counseling, and care planning for hospice patients and families in Newark, OH. This role involves home visits, collaboration with an interdisciplinary team, and guiding families through advance directives and end‑of‑life planning.

The position offers weekday hours with shared weekend rotation and regular travel to patient homes and facilities in Newark and surrounding communities.

Qualifications

• Bachelor’s degree in Social Work (BSW)
• Current Ohio LSW or LISW license
• Minimum 1 year of healthcare experience
• Valid driver’s license
• Reliable transportation and required automobile insurance
• Excellent communication, organization, and documentation skills
• Ability to work independently while collaborating effectively with an interdisciplinary team

Responsibilities

• Assess psychosocial, emotional, financial, and practical needs of patients and families
• Develop individualized plans of care based on patient goals and preferences
• Provide counseling, education, emotional support, and crisis intervention as appropriate
• Connect patients and families with financial assistance, community resources, agencies, and support programs
• Assist patients and families with advance directives and end‑of‑life planning
• Provide caregiver support related to stress, anticipatory grief, and bereavement
• Advocate for patient rights, preferences, dignity, comfort, and quality of life
• Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, and other care professionals
• Complete accurate documentation in the EMR
• Provide compassionate, patient‑centered care in homes, nursing facilities, and assisted living communities
