Job summary

SSM Health is seeking a PRN Registered Nurse for Hospice in Mount Vernon, Illinois. The role requires traveling to patient homes, CB RFs, and other locations to assess care needs and coordinate the plan of care under the attending physician’s direction.

Responsibilities include discharge planning, skilled nursing tasks, documentation, and collaboration with the care team to ensure quality end-of-life care. A valid RN license and BLS are required; multiple state locations are supported where

Qualifications

• Graduate of accredited school of nursing or education equivalency for licensing.
• No experience required.
• Proof of motor vehicle liability insurance that meets minimum state requirements upon hire and upon request.

Responsibilities

• Coordinate, facilitate and perform interdisciplinary plan of care through assessment and evaluation.
• Provide skilled nursing care per plan of care including vitals, wound, IV, ostomy care and physical assessment.
• Communicate with physician and care team; document updates and revisions of the care plan.
• Complete admission paperwork and verify insurance information; ensure timely patient assessments.
• Document visit notes and ensure timely, accurate documentation in accordance with regulations.
