Qualifications

• Graduate of accredited school of nursing or education equivalency for licensing.

Responsibilities

• Assesses and coordinates patient's discharge planning needs with healthcare team.
• May perform point of care testing according to policies and procedures.
• Collaborates with physician on plan of care and updates on progress; documents status per regulations.
• Coordinates with multi-disciplinary team and outside agencies to meet outcomes and document care.
• Completes admission paperwork and comprehensive medical assessment in clinical software.
• Provides skilled nursing care per the care plan: vitals, wound/IV/ostomy care, meds, and exams.
• Documents visit notes and treatments; ensures visits are timely and properly recorded.
• Engage with patient/family to develop and adjust the care plan; respond to calls and follow up.
