Job summary

SSM Health at Home Janesville is seeking a PRN Registered Nurse to join our Hospice team. You will assess patients, coordinate care plans, and travel to patient homes, CB RFs, assisted living locations, and hospitals to meet patient and family needs under the direction of the attending physician.

In this role you will perform skilled nursing tasks, document visits, support families, and collaborate with the interdisciplinary team to ensure safe, effective end-of-life care.

Qualifications

• RN license in good standing (WI or compact)
• BLS HCP certification (AHA)
• Ability to travel to patient homes within assigned territory
• Home health/hospice experience preferred

Responsibilities

• Assess and coordinate patients' discharge planning needs with the healthcare team
• Perform skilled nursing tasks (vital signs, wound care, IV care, ostomy care, injections)
• Communicate with physicians regarding plan of care and patient status
• Document visits, treatments, and care in the chart per regulations
• Collaborate with patient/family and care team to develop a comprehensive care plan
• Ensure timely documentation and required visits are completed
