Transitions Hospice is seeking a Registered Nurse for visit-based care in Buffalo Grove, IL. This PRN role supports an Extended Care Team model, providing flexible, compassionate nursing across multiple patients and settings.

As an RN with hospice or home health experience, you will assess, treat, and document patient needs promptly, administer medications per orders, and coordinate with families and physicians to ensure seamless care. Local travel and adaptability are essential qualities.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or related setting preferred.
• Flexibility and confidence working across varied patient assignments.
• Strong assessment skills and sound clinical judgment under pressure.
• Excellent documentation habits and communication skills.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct comprehensive patient and family assessments and contribute to care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health data and respond accordingly.
• Record timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and services are arranged and available for patients.
• Maintain and grow clinical skills through continuing education.
