Job summary

Ennoble Care in Port Charlotte, FL is seeking an experienced Full-time Provider (Nurse Practitioner or Physician Assistant) to provide primary and palliative care to our patients at home, under physician supervision.

You will perform physical examinations, develop care plans, order diagnostic testing, and prescribe medications. You will collaborate with our in‑home clinical team to deliver patient‑centered care and participate in quarterly medical staff meetings.

Qualifications

• Board Certified, with a valid NP or PA certification in the State of Florida.
• 1–2 years of primary care experience with geriatric patients.

Responsibilities

• Provide primary and palliative care to patients in Port Charlotte, FL.
• Perform physical examinations and develop care plans.
• Communicate with office staff and clinical operations across platforms.
• Provide patient-centered care to a panel of patients.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnosis, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary and prescribe medications.
