Ennoble Care seeks an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in the Philadelphia region on a full-time basis. You will work under a physician supervising and care for patients with chronic diseases in a home setting.

Board certified with PA/NP license in PA and strong independent clinical skills are required. Hospice experience is valued but not mandatory; a flexible, autonomous role with in-house

Formación

• Board Certified with a valid PA or NP license in PA.
• Strong clinical skills to assess, diagnose and treat independently.
• 1–2 years of primary care with geriatric patients; hospice experience preferred.

Responsabilidades

• Provide primary and palliative care to homebound patients in the Philadelphia region.
• Perform physical exams and update medical histories; formulate care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Deliver patient-centered care to a defined panel of patients.
• Develop impressions, diagnoses and treatment plans.
• Build trust with patients, families and caregivers during visits.
• Order necessary diagnostic tests.
• Prescribe medications and necessary interventions.
