Ennoble Care is seeking an experienced Advanced Provider (NP/PA) to deliver primary and palliative care to homebound patients in the Philadelphia, PA region on a full-time basis. You will apply medical techniques under physician supervision for a diverse patient population with chronic diseases.

Responsibilities include exams, histories, care plans, and collaboration with staff. Requires PA/NP certification in PA, strong clinical skills, and 1–2 years in primary care with geriatric patients.

Qualificações

• Board Certified with a valid NP or PA certification in PA.
• Strong clinical skills to diagnose, treat and manage care.
• Positive demeanor and excellent interpersonal skills.
• Ability to problem solve and operate independently.
• 1–2 years of primary care experience with geriatric patients.

Responsabilidades

• Provide high quality primary and palliative care in the Philadelphia, PA region.
• Perform physical examinations and update patient medical histories.
• Communicate effectively with office staff across platforms.
• Deliver patient centered care to a panel of patients.
• Develop impressions, diagnoses and treatment plans.
• Consult with patients, families and caregivers during visits.
• Order diagnostic tests as medically necessary.
• Prescribe medications and medically necessary interventions.
