Job summary

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, seeks Registered Nurses to join our award-winning team. We deliver compassionate care at home and support patient independence through personalized care plans.

Qualified candidates hold a current RN license, BLS certification, and at least 1 year of home health or healthcare facility experience. Strong communication and a valid driver’s license are also required.

Qualifications

• Current RN license (state-specific).
• Valid BLS certification.
• Minimum 1 year of experience in home health or a healthcare facility.
• Familiarity with Medicare and Medicaid guidelines.
• Strong communication, organizational, and time management skills.

Responsibilities

• Design and implement patient-specific care plans.
• Provide nursing care aligned with the care plan and conduct assessments.
• Monitor patient conditions and coordinate health promotion and direct care.
• Maintain detailed clinical records and required OASIS documentation.
• Communicate with clinical managers and colleagues to ensure quality care.
• Provide training and support to patients and families/caregivers.
• Be a liaison between patients and community health resources.
• Collaborate with physicians and discharge planners on health education.
• Confirm daily visit schedules for seamless coordination.
• Engage in the agency’s QAPI program and address complaints as needed.
• Uphold patient rights per policies.
