This full-time RN role emphasizes thorough assessments, care plan implementation, and coordination with physicians and families.

Qualifications

• Current RN license (state-specific) and valid BLS certification.
• Minimum of 1 year of experience in home health or healthcare facility.
• Familiarity with Medicare/Medicaid guidelines.
• Excellent communication, organizational, and time management skills.
• Valid driver’s license and access to a reliable vehicle.

Responsibilities

• Design and implement patient-specific care plans to support recovery.
• Provide nursing care aligned with the patient’s care plan and complete assessments.
• Monitor patient conditions and coordinate health promotion and direct care.
• Maintain accurate clinical records and required OASIS documentation.
• Communicate with clinical managers and colleagues to ensure quality care.
• Provide training and support to patients and families/caregivers.
• Serve as a liaison between patients and community health resources.
• Coordinate with physicians and discharge planners for health education.
• Confirm daily visit schedules to ensure seamless coordination.
• Engage with QAPI programs and contribute as needed.
• Uphold patient rights per policies.
