Job summary

Pinnacle Home Care in Florida is seeking a dedicated Registered Nurse to join our home health team. You will design care plans, deliver bedside nursing, and coordinate with physicians, caregivers, and management to support patient recovery at home.

The ideal candidate has an active RN license, BLS, and at least one year of home health or healthcare facility experience. A valid driver’s license and reliable vehicle are required.

Qualifications

• Current RN license (state-specific).
• Valid BLS certification.
• Minimum one year of experience in home health or a healthcare facility.
• Familiarity with Medicare and Medicaid guidelines.
• Solid understanding of professional nursing principles and home health documentation.
• Excellent communication, organizational, and time management skills.
• Must have a valid driver’s license and access to an insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform evaluations and care techniques.

Responsibilities

• Design and implement patient-specific care plans focused on recovery.
• Provide nursing care aligned with care plans and conduct health assessments.
• Monitor patient conditions and coordinate health promotion and direct care.
• Maintain detailed clinical records and OASIS documentation.
• Communicate with clinical managers and colleagues to ensure high-quality care.
• Provide training and support to patients and families/caregivers.
• Act as liaison between patients and community health resources.
• Coordinate with physicians and discharge planners for health education.
• Confirm daily visit schedules to ensure seamless coordination.
• Engage in QA/QIP programs and address customer feedback appropriately.
• Respect patient rights and adhere to admission/discharge policies.
