Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, seeks Registered Nurses to join a mission-driven team delivering compassionate care at patients’ homes. You will design personalized care plans, assess health and psychosocial needs, and coordinate services with physicians and discharge planners.

Ideal candidates have an active RN license, BLS, and at least one year of home health or hospital experience, with strong communication and organizational skills.

Competenze

• Must have a current RN license (state-specific).
• BLS certification required.
• Minimum one year of experience in home health or a healthcare facility.
• Familiarity with Medicare and Medicaid guidelines.
• Solid understanding of professional nursing principles and home health documentation.
• Excellent communication, organizational, and time management skills.
• Must have a valid driver’s license and reliable vehicle.
• Ability to visit homes that may not be wheelchair accessible.

Mansioni

• Design and implement patient-specific care plans focused on recovery.
• Provide nursing care aligned with the patient’s care plan and assessments.
• Monitor patient conditions and coordinate health promotion and direct care.
• Maintain detailed clinical records, including OASIS documentation.
• Communicate with clinical managers and colleagues to ensure quality care.
• Provide training and support to patients and families/caregivers.
• Act as a link between patients and community health resources.
• Collaborate with physicians and discharge planners on health education.
