Pinnacle Home Care in Florida is seeking a Registered Nurse to join our award-winning team. You will design patient-specific care plans and provide hands-on nursing care in the patient’s home, coordinating with physicians and discharge planners.

This role requires an active RN license, BLS, at least one year in home health or healthcare; strong communication and organizational skills are essential. We offer CEUs, flexible scheduling, and a supportive culture.

Competenze

• Must hold a current RN license (state-specific).
• Valid BLS certification.
• Minimum one year of home health or healthcare facility experience.
• Familiarity with Medicare and Medicaid guidelines.
• Strong nursing principles and home health documentation knowledge.
• Excellent communication, organizational, and time management skills.
• Valid driver’s license and access to a reliable insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Ability to perform physical assessments and care techniques.

Mansioni

• Design and implement patient-specific care plans focused on recovery and health.
• Provide nursing care per the care plan and conduct comprehensive assessments.
• Monitor patients, coordinate health promotion and direct care.
• Maintain detailed clinical records including OASIS documentation.
• Communicate with clinical managers and colleagues to ensure high-quality care.
• Provide training and support to patients and families.
• Serve as a key contact between patients and community health resources.
• Collaborate with physicians and discharge planners to educate patients and caregivers.
• Confirm daily visit schedules for seamless coordination.
• Participate in the agency’s QAPI program and address issues as required.
• Uphold patient rights per service agreements and admission policies.
