Job summary

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking a Registered Nurse (RN) for full-time work at patients' homes across Florida. The role focuses on designing patient-specific care plans, performing comprehensive assessments, and coordinating care with a multidisciplinary team.

Ideal candidates hold an active RN license and BLS, with at least one year in home health or a healthcare setting.

Qualifications

• Current RN license (state-specific) required.
• Valid BLS certification required.
• Minimum one year of home health or facility experience.
• Familiarity with Medicare and Medicaid guidelines.
• Strong communication, organization, and time management.
• Valid driver's license and access to insured vehicle.
• Ability to visit homes, including non-wheelchair accessible.
• Must have adequate hearing, vision, and dexterity for assessments.
• Able to perform physical tasks including walking, standing, lifting.

Responsibilities

• Design and implement patient-specific care plans.
• Provide nursing care per the care plan and perform assessments.
• Monitor patient conditions and coordinate care.
• Maintain accurate clinical records including OASIS.
• Communicate with clinical managers and staff.
• Provide training and support to patients and families.
• Serve as a liaison to community health resources.
• Collaborate with physicians and discharge planners on education.
• Confirm daily visit schedules for seamless coordination.
• Participate in QAPI program and address complaints.
• Uphold patient rights per policies.
