Job summary

Elara Caring is seeking a Home Health LPN on a PRN basis to deliver skilled nursing care in patients’ homes across Beverly, Peabody, Malden, and surrounding communities. You’ll work under the direction of an RN and physician, use individualized plans of care, and support one-on-one patient care with a compassionate, clinical team.

Qualifications

• Current Massachusetts LPN license.
• Graduate of an accredited nursing program.
• At least one year of home care experience.
• Valid driver’s license and reliable transportation.
• Ability to travel throughout Malden, Lowell, and surrounding communities.
• Home health or hospice experience preferred.

Responsibilities

• Provide skilled nursing care under the direction of an RN and physician.
• Follow individualized plans of care and physician orders.
• Monitor patients and report changes to the RN Case Manager.
• Educate patients and caregivers on medications, treatments, and self-care.
• Complete accurate and timely clinical documentation.
• Collaborate with the care team to improve outcomes and help prevent avoidable hospital visits.

$32–$38 per hour, based on experience
