Elara Caring in Palmer–Monson, MA is seeking a Licensed Practical Nurse (LPN) to perform in-home visits and coordinate care with an RN Case Manager. Schedule is Monday–Friday, 8 a.m.–5 p.m., with pay ranging from $30–$38 per hour based on experience.

The role emphasizes compassionate, individualized patient care, documentation, and collaboration with the clinical team to support patient independence and safety.

Formación

• Active Massachusetts LPN license is required.
• Graduation from an accredited nursing program.
• At least one year of clinical nursing experience.
• Valid driver’s license and reliable transportation.
• Ability to travel within Palmer–Monson territory.
• Ability to lift 50–100 lbs during shifts.

Responsabilidades

• Provide skilled nursing care per patient care plan and orders.
• Monitor patient progress and report changes to RN Case Manager.
• Educate patients and caregivers on self-care techniques.
• Collaborate with the RN Case Manager on care plans.
• Document visits and medications accurately.
• Coordinate care with physicians, families, and clinical team.
• Help reduce avoidable hospitalizations through attentive care.

Monday–Friday, 8 a.m.–5 p.m. | $30–$38/hour, based on experience

What You’ll Do

• Provide skilled nursing care according to each patient’s individualized care plan and physician orders.
• Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
• Teach patients and caregivers self-care techniques that support health, safety, and independence.
• Collaborate with the RN Case Manager as patient needs and care plans change.
• Document visits, nursing interventions, and medication information accurately and promptly.
• Communicate with physicians, families, and the clinical team to keep care coordinated.
• Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.

What You’ll Bring

• An active Massachusetts LPN license
• Graduation from an accredited nursing program
• At least one year of clinical nursing experience
• A valid driver’s license, reliable transportation, and current auto insurance
• Ability to travel throughout the Palmer–Monson territory
• Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
