Job summary

Elara Caring in Michigan is seeking a Licensed Practical Nurse (LPN) for Full Time Home Health to support patients in their homes. You will follow care plans, coordinate with RNs, and monitor patient progress.

This role emphasizes compassionate, high-quality care and proactive patient engagement. The position requires a valid LVN/LPN license, nursing education, at least 1 year of clinical experience, and the ability to lift up to 50 pounds with reliable transportation to travel between patient

Qualifications

• Graduate of accredited nursing program (Certificate/Diploma/Associate).
• 1 year of clinical care experience preferred.
• Must be able to lift and move patients/equipment up to 50 pounds.
• Reliable transportation to travel between patient homes.

Responsibilities

• Follow patient care plans and physician orders.
• Monitor patient progress and report changes to care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with RN Case Manager as needs change.
• Document medications and update profiles per guidelines.
• Communicate with physicians, patients, families, and team for coordinated care.
• Help prevent unnecessary hospital visits through proactive care.
• Deliver compassionate, high-quality care.

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
