Job summary

Addus HealthCare is seeking an RN to provide in-home patient care with a flexible schedule. You will supervise care plans, complete Starts of Care, and work closely with case managers to ensure high-quality service while supporting patient independence.

The role requires RN licensure and at least 2 years of nursing experience, with the ability to travel locally in your own vehicle and maintain appropriate insurance coverage.

Qualifications

• Graduation from an accredited school of nursing with current RN licensure.
• Minimum of 2 years nursing experience.
• Ability to travel locally in your own vehicle with valid driver's license and auto insurance.

Responsibilities

• Enable patients to stay independent in their own homes.
• Perform routine supervisory visits to evaluate the care plan and service delivery.
• Complete initial Starts of Care.
• Deliver educational in-service trainings.
• Communicate with Case Managers and other care-team members.

As an Addus Nurse (Registered Nurse), you will:

• Enable patients to stay independent while they remain in their own homes.
• Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service.
• Complete initial Starts of Care.
• Present educational in-service trainings
• Communicate with Case Managers and other members of the care team

REQUIREMENTS

• Graduation from an accredited school of nursing and current RN licensure.
• 2 years of nursing experience.
• Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage.
