Job summary

Addus HealthCare is seeking a Registered Nurse to help patients stay independent at home. You will work with a flexible schedule, including self scheduling, with no weekends or holidays.

The role requires supervising the plan of care, initiating Starts of Care, and delivering education to the care team while coordinating with Case Managers. A valid RN license and at least 2 years of nursing experience are required, plus local travel in your own vehicle.

Qualifications

• RN licensure current and in good standing.
• Minimum of 2 years of nursing experience.
• Ability to travel locally in own vehicle with valid driver's license and auto insurance.

Responsibilities

• Enable patients to stay independent while they remain in their own homes.
• Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service.
• Complete initial Starts of Care.
• Present educational in-service trainings
• Communicate with Case Managers and other members of the care team

REQUIREMENTS

• Graduation from an accredited school of nursing and current RN licensure.
• 2 years of nursing experience.
• Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage.
