Job summary

Addus HomeCare is hiring a Registered Nurse to provide in-home patient care, supervise plans of care, and support clinical teams. You will complete Starts of Care and coordinate with case managers to ensure high-quality home health services.

Ideal candidates hold an accredited nursing degree with RN licensure and have at least two years of nursing experience. Local travel in your own vehicle is required, along with a valid driver's license and insurance.

Qualifications

• Graduation from an accredited school of nursing and RN licensure.
• Minimum 2 years of nursing experience.
• Must travel locally in own vehicle with valid driver's license and insurance.

Responsibilities

• Enable patients to stay independent in their own homes.
• Perform routine supervisory visits to evaluate the plan of care and supervise client service delivery.
• Complete initial Starts of Care.
• Communicate with Case Managers and care team members.

As an Addus Nurse (Registered Nurse), you will:

• Enable patients to stay independent while they remain in their own homes.
• Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service.
• Complete initial Starts of Care.
• Communicate with Case Managers and other members of the care team

REQUIREMENTS

• Graduation from an accredited school of nursing and current RN licensure.
• 2 years of nursing experience.
• Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage.
