Job summary

Addus HomeCare is seeking a Registered Nurse to support patients in their homes, helping them stay independent and receive high-quality care.

Responsibilities include supervision of care plans, starts of care, coordinating with case managers, and delivering excellent client service; travel locally with your own vehicle is required.

Qualifications

• RN licensure from an accredited nursing program.
• Minimum of 1 year nursing experience.
• Must be able to travel locally by own vehicle with valid driver’s license and auto insurance.

Responsibilities

• Provide in-home patient care and support independence.
• Supervise care plans and ensure high-quality client service.
• Complete starts of care and coordinate with case managers.
• Communicate effectively with the care team to meet patient needs.

As an Addus Nurse (Registered Nurse), you will:

• Enable patients to stay independent while they remain in their own homes.
• Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service.
• Complete initial Starts of Care.
• Communicate with Case Managers and other members of the care team

REQUIREMENTS

• Graduation from an accredited school of nursing and current RN licensure.
• One year of nursing experience.
• Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage.
