Job summary

Autumn Lake at Union in Union, NJ is seeking a PRN Certified Occupational Therapy Assistant to support skilled nursing residents by implementing the OT plan of care and promoting independence.

You will work with the OT, assist with dressing, grooming, and functional tasks, document progress, and collaborate with nursing and rehab staff; NJ license and COTA required; SNF experience helpful.

Qualifications

• Active New Jersey OT license and COTA certification.
• Ability to follow an established plan of care.
• Clear communication and accurate documentation.
• New graduates welcome; SNF experience helpful.

Responsibilities

• Carry out the OT plan of care through individualized treatment activities.
• Coach residents through dressing, grooming, toileting, and bathing tasks.
• Lead activities to develop upper-extremity function and coordination.
• Reinforce adaptive equipment use for safe task completion.
• Share observations with the OT about progress and concerns.
• Complete treatment notes and collaborate with nursing and rehab staff.
