Responsibilities

Job Summary:

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by a physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care. The nurse works under supervision of a Registered Nurse or designated supervisor, adhering to state laws, licensure requirements, and agency policies.

Specific Duties

• Deliver high-quality clinical services within legal scope, following the care plan and infection control procedures.
• Thoroughly document patient visits in the electronic medical record, complying with regulatory requirements.
• Participate in care coordination, report patient needs and changes promptly, and communicate effectively with patients, families, and healthcare team members.
• Maintain confidentiality of patient information per agency policy.

Education and Experience

License Requirements: Must hold a current LPN license in Tennessee. Valid driver’s license, vehicle insurance, access to dependable transportation, and current CPR certification are required.

Experience Requirements: Previous home care experience preferred.
