Job Summary

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes, records clinical and progress notes, and coordinates other patient care activities/services to ensure high-quality care. Works under the supervision of the Registered Nurse and/or designated supervisor, within the scope of practice defined by state laws and agency policies.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within the scope of practice, in accordance with the plan of care, and in coordination with the healthcare team, following infection control procedures.
• Documents patient visits thoroughly and accurately in the electronic medical record, utilizing the agency's electronic visit verification program.
• Participates in care coordination, reports patient needs and changes in condition, attends case conferences, and communicates effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient information per agency policy.

License Requirements

• Current LPN licensure in Tennessee
• Valid driver's license, vehicle insurance, and access to dependable transportation
• Current CPR certification

Experience Requirements

• Previous home care experience preferred
