Job Summary

The Home Care Licensed Practical Nurse (LPN) is responsible for providing direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes. Responsibilities include recording clinical notes, coordinating patient care activities, and working under the supervision of a Registered Nurse or designated supervisor. Activities are limited to those in compliance with state laws, licensure requirements, and agency policies.

Responsibilities

1. Provide high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
2. Document patient visits thoroughly in the electronic medical record and electronic visit verification system.
3. Participate in care coordination, report patient needs and changes promptly, and communicate effectively with patients, families, and healthcare team members.
4. Maintain confidentiality of patient information per agency policy.

Requirements

• Current LPN licensure in Tennessee.
• Valid driver’s license, vehicle insurance, and access to dependable transportation.
• Current CPR certification.
• Preferred: Previous home care experience.
