Job Summary

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes, coordinating patient care activities, and working under the supervision of a Registered Nurse or designated supervisor. Activities are limited to those within the scope of practice defined by state laws and agency policies.

Specific Job Duties

1. Provides high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
2. Documents patient visits thoroughly in the electronic medical record.
3. Participates in care coordination, reports patient needs and changes, and communicates effectively with patients, families, and healthcare team members.
4. Maintains confidentiality of patient information per agency policy.

Qualifications

• Must have current LPN licensure in Tennessee.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
• Previous home care experience preferred.
