Responsibilities

Job Summary

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by a physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality service within the geographic area. The nurse works under supervision of a Registered Nurse or designated supervisor, adhering to state laws, licensure requirements, and agency policies.

Specific Job Duties/Responsibilities

• Deliver high-quality clinical services within the scope of practice, following the care plan and infection control procedures.
• Thoroughly document patient visits in the electronic medical record, complying with regulatory and payer requirements.
• Participate in care coordination, report patient needs and changes, attend case conferences, and communicate effectively with patients, families, physicians, and team members.
• Maintain confidentiality of patient information per agency policy.

Education and Experience

Education Requirements

• Meet educational and experience requirements as per state licensure laws.

Experience Requirements

• Minimum one year of acute care experience preferred.
• Previous home care experience preferred.

License Requirements

• Current LPN license in the practicing state.
• Valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation.
• Current CPR certification.
