Job Summary

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes, records clinical notes, and coordinates patient care activities to ensure high-quality service. Works under supervision of RN or designated supervisor, within scope of practice per state laws and agency policies.

Specific Job Duties

• Provides high-quality clinical services following the plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination, reports patient needs and changes, communicates effectively with team members.
• Maintains confidentiality of patient information.

License Requirements

• Current LPN licensure in Tennessee.
• Valid driver’s license, vehicle insurance, and dependable transportation.
• Current CPR certification.

Experience Requirements

• Previous home care experience preferred.
