Responsibilities

Job Summary: The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes, records clinical notes, and coordinates patient care activities to ensure high-quality care. Works under supervision of the RN or designated supervisor, within scope of practice per state laws and agency policies.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice, following the plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record and electronic visit verification system.
• Participates in care coordination, reports patient needs and changes, attends case conferences, and communicates effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient information per policy.

Qualifications

Education and Experience:

• Current LPN licensure in Tennessee.
• Valid driver’s license, vehicle insurance, and access to transportation.
• Current CPR certification.
• Previous home care experience preferred.
