Job Summary

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes, coordinating patient care activities, and ensuring high-quality care within the scope of practice, in compliance with state laws and agency policies. Works under supervision of RN or designated supervisor.

Specific Job Duties

• Provide high-quality clinical services within legal scope, following the plan of care and infection control procedures.
• Document patient visits thoroughly in the electronic medical record.
• Participate in care coordination, report patient needs and changes, and communicate effectively with patients, families, and team members.
• Maintain patient confidentiality per agency policy.

License Requirements

• Current LPN licensure in Tennessee
• Valid driver’s license, vehicle insurance, and access to transportation
• CPR certification

Experience Requirements

• Preferred: previous home care experience
