Responsibilities

Job Summary: The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality service within the assigned geographic area. The LPN works under the supervision of a Registered Nurse or designated supervisor, adhering to state laws, licensure requirements, and agency policies.

Specific Duties:

• Deliver high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
• Thoroughly document patient visits in the electronic medical record, complying with regulatory and payer requirements.
• Participate in care coordination, report patient needs and changes promptly, attend case conferences, and communicate effectively with patients, families, physicians, and team members.
• Maintain confidentiality of patient information per agency policy.

Education and Experience

License Requirements: Must hold a current LPN license in Tennessee, possess a valid driver’s license, vehicle insurance, and have access to a dependable vehicle or public transportation. Current CPR certification is required.

Experience Requirements: Previous home care experience is preferred.
