Responsibilities
Job Summary
The Home Care Licensed Practical Nurse is responsible for providing direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes, recording clinical and progress notes, and coordinating other patient care activities/services to ensure the highest quality of care in the assigned geographic area. She/he works under the supervision of the Registered Nurse or designated supervisor. Nursing activities are limited to those in compliance with the State LPN/LVN Practice Act, licensure/certification requirements, and agency policies.

Education and Experience
License Requirements
Must have a current LPN license in Tennessee. Must possess a valid driver's license, vehicle insurance, and have access to a dependable vehicle or public transportation. Current CPR certification required.
Experience Requirements
Previous home care experience preferred.
