Responsibilities

• Perform nursing procedures within the scope of LPN practice
• Educate patients and families as outlined by the RN
• Monitor vital signs and document observations
• Record intake and output, reporting significant changes
• Assess and document patient’s physical and mental states
• Administer medications as ordered by the physician and instructed by the RN
• Participate in staff meetings and in-service training

Qualifications

• Active LPN license in good standing
• CPR Certification
• Previous experience in home health, private duty, skilled nursing, or hospital setting preferred
• Strong rapport-building skills with patients and families
• Problem-solving and critical thinking skills
• Reliable transportation or valid driver’s license with current insurance
