Job responsibilities include, but are not limited to:

• One-on-One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for the patient
• Administration of prescribed medication, treatments, and therapies
• Patient assessments; coordination of care
• Report any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
