Job summary

The incumbent serves as Primary Care Provider within the Patient Aligned Care Team (PACT) at a VA facility in Memphis, overseeing a patient panel, leading a team, and guiding care using performance measures and PDSA cycles.

Responsibilities include supervising residents, collaborating with mid-level practitioners, and providing telehealth and in-person care. The VA offers competitive pay, comprehensive benefits, and Education Debt Reduction Program options.

Qualifications

• Must hold a current full and unrestricted license to practice medicine in the U.S.
• Completion of an accredited residency program in a core specialty
• Board certification (eligible for faculty status) or equivalent training
• Proficiency in spoken and written English

Responsibilities

• Provide direct patient care for a panel using the PACT model
• Lead the Primary Care Teamlet, participate in performance improvement initiatives
• Collaborate with mid-level practitioners and supervise residents or students as applicable
• Engage in team huddles, care coordination, and utilization of telehealth and other modalities
• Participate in staff meetings and fulfill credentialing and regulatory requirements
• Support performance measurement goals and participate in program reviews
