Job summary

The Veterans Health Administration seeks a Physician for a Patient Aligned Care Team (PACT) at the Hamilton CBOC. This role provides direct primary care to enrolled veterans as part of a multi-disciplinary team including RN, LPN, and clerical staff.

The incumbent will perform examinations, diagnose and treat acute and chronic conditions, and coordinate care with specialists. The position is full-time with a comprehensive federal benefits package and opportunities for teaching and leadership

Qualifications

• Must hold an MD or DO degree from an approved school.
• Current, full and unrestricted medical license in the US.
• Board certification or eligibility in internal medicine or family medicine is preferred.

Responsibilities

• Provide clinical examination, diagnosis, and treatment of acute and chronic diseases.
• Deliver preventive care and coordinate care for a panel of primary care patients.
• Document patient care in medical records and refer to specialists as needed.
• Monitor PACT team performance to ensure quality and VA performance measures.
