Job summary

The Cincinnati VA Medical Center, part of Veterans Health Administration, is seeking a Primary Care Physician to lead a Patient Aligned Care Team (PACT) delivering comprehensive care to Veterans at the main hospital or Community Based Outpatient Clinics. The PCP provides clinical examination, diagnosis, and treatment, coordinates care, and ensures documentation.

Qualifications

• Degree of Doctor of Medicine or an equivalent degree in allopathic/osteopathic medicine.
• If foreign medical graduates, ECFMG certification verification may be required.

Responsibilities

• Lead a Patient Aligned Care Team (PACT) and provide direct care to Veterans.
• Perform clinical examinations, diagnose and treat acute and chronic illnesses.
• Coordinate care and ensure proper documentation in medical records.
• Refer to specialists when needed and monitor team performance against VA standards.
• Oversee patient concerns and inquiries with a focus on service excellence.
