ChenMed LLC is hiring Primary Care Physicians (PCP) to deliver high-quality, value-based care for seniors in our physician-led, mission-driven network. The PCP will manage a panel of up to 450 patients, uphold outcomes, and lead care teams in a collaborative, patient-centered environment.

We seek innovators who can coach patients, coordinate care across hospitalists and specialists, and drive performance metrics while adhering to guidelines and compliance.

Qualificações

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics required.
• English fluency required and active state medical license.
• Experience in value-based care and leading care teams is preferred.

Responsabilidades

• Provide care for adults with acute and chronic illnesses.
• Lead care teams and coordinate with hospitalists and specialists.
• Meet quality, cost and regulatory standards and document care accurately.
