Job summary

Cityblock Health is seeking a Pod Provider who can operate remotely from Cincinnati or Columbus, with weekly visits to the hub location. The role delivers comprehensive chronic disease management and preventive care while collaborating with an interdisciplinary team to develop member care plans.

You will conduct intake visits, support care transitions, manage hypertension, diabetes, and mental health concerns, and contribute to clinical programs and policies.

Qualifications

• 3+ direct clinical care experience.
• Completed NP or PA program with national certification.
• Current valid license to practice medicine in the state of employment.
• DEA licensure (or willingness to acquire).
• Certified in Basic Life Support (BLS) for Healthcare Providers.

Responsibilities

• Conduct comprehensive clinical intake visits and provide longitudinal primary care with emphasis on chronic condition management and preventive care.
• Proactively outreach to members needing follow-up care and ensure proper coding for diagnoses (ICD-10) and care planning.
• Prescribe first and second line medications for depression, anxiety, alcohol use disorders, and opioid use disorders; support care transitions.
• Stay up-to-date on evidence-based care for complex patients; refer to specialists when appropriate and support member goals across physical, behavioral, and social health.
• Participate in case conferences and collaborate on care plans and panel management.
