Conviva Senior Primary Care, a Humana subsidiary, seeks a Primary Care Physician for our Ormond Beach, FL clinic. You will report to the Associate Medical Director and be on-site, working Monday through Friday from 8:00 am to 5:00 pm, seeing about 15 patients per day and conducting longer 45-minute visits.

Requirements include MD/DO, board certification or eligibility in geriatrics, family, or internal medicine, Florida license, DEA, TB screening, 2+ years' experience, and Medicare Advantage

Competenze

• Doctor of Medicine or Osteopathic Medicine degree required.
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine.
• Valid, unrestricted license in Florida or able to obtain one.
• Current, unrestricted DEA license.
• TB screening completed.
• Minimum of two years of experience.
• Experience managing a Medicare Advantage panel preferred.
• Language proficiency assessment may be required for non-English speakers.

Conviva Primary Care, a subsidiary of Humana Inc., is seeking a primary care physician for our clinic in Ormond Beach, FL.

As a Primary Care Physician, you will report to the Associate Medical Director. You will be on-site at the Ormond Beach clinic.

A day in the life of a Conviva physician

• Monday-Friday 8:00 am-5:00 pm
• See an average of 15 patients per day
• Longer authorized appointments (45 minutes)
• You will be on-site

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of Florida, or able to obtain one.
• Current, unrestricted DEA license
• Screened for TB
• Minimum of two years of experience
• One or more years of experience managing a Medicare Advantage panel of patients
• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government. #LI-FM1

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
