Job summary

Conviva Primary Care, a Humana Inc. subsidiary, is seeking a Primary Care Physician for our Jacksonville, FL clinic in Callahan. You will be on-site and report to the Associate Medical Director, delivering high-quality, value-based care to adults in a welcoming setting.

The position is full-time, Monday through Friday, 40 hours. The day typically includes 12–15 patient visits with longer 45-minute appointments.

Qualifications

• MD/DO with board eligible or board certified in Geriatrics, Family Medicine, or Internal Medicine.
• Active, unrestricted medical license in the state of employment or willingness to obtain.
• Current, unrestricted DEA license.
• Minimum 2+ years in value-based care.
• 1+ year managing Medicare Advantage panels.
• Language proficiency assessment for non-English speakers.

Responsibilities

• See 12-15 patients per day with longer 45-minute appointments.
• On-site at the Callahan, FL clinic and report to the Associate Medical Director.
• Provide primary care services focused on seniors within Conviva Senior Primary Care.

Job description

Become a part of our caring community Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Jacksonville, FL.

As a Primary Care Physician, you will report to the Associate Medical Director. You will be onsite at the Callahan, FL. clinic.

A day in the life of a Conviva physician

Monday-Friday 8:00am-5:00pm See 12-15 patients per day Longer authorized appointments (45 minutes) You will be on-site.

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of desired employment (or willing to obtain)
• Current, unrestricted DEA license
• Screened for TB
• Minimum of two+ years of experience in value base care
• 1+ years of experience managing Medicare Advantage panel of patients
• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
