Job summary

Conviva Primary Care, a Humana subsidiary, seeks a board-certified or board-eligible primary care physician to join our onsite clinic in Plantation, FL. You will provide comprehensive outpatient care to adults and seniors, maintain medical records, and coordinate with interdisciplinary teams in a patient-centered, whole-person approach.

Working Monday–Friday, 8:00am–5:00pm, you will benefit from a collaborative culture, opportunities for CME, and a competitive compensation package with a pay

Qualifications

• MD or DO degree from an accredited medical school.
• Board certification or eligibility in family medicine, internal medicine, or geriatric medicine.
• Valid, unrestricted Florida medical license (or willingness to obtain).
• Current unrestricted DEA license (or ability to obtain).
• Two years of experience in primary, geriatric, or chronic-care management.
• Bilingual in Spanish and English is a plus.

Responsibilities

• Deliver comprehensive outpatient care to adults and seniors.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Collaborate with interdisciplinary teams to ensure holistic care.

You will work onsite at the Conviva clinic in Plantation, FL and report to the associate medical director.
Monday-Friday 8:00am-5:00pm

Required Qualifications

• Two years' or more experience in primary care, geriatric care, or managing chronic conditions
• Bilingual in Spanish and English a plus

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$255,000 - $298,500 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
