Conviva Primary Care, a Humana Inc. subsidiary, seeks a Primary Care Physician for our Daytona, FL clinic. On-site Monday–Friday, 8:00 am–5:00 pm, with about 15 patients daily and 45‑minute appointments.

Qualifications

• MD/DO required with board certification or eligibility in geriatrics, family, or internal medicine.
• Active Florida license or eligibility to obtain one and a valid DEA license.
• TB screening completed as part of pre-employment process.
• Minimum 2 years of clinical experience, with 1+ year managing a Medicare Advantage panel.
• Non‑English speaking members require language proficiency assessment; ILR testing administered.

Become a part of our caring community Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Daytona, FL. As a Primary Care Physician, you will report to the Associate Medical Director. You will be on-site at our Daytona, FL. clinic.

A day in the life of a Conviva physician Monday-Friday, 8:00 am-5:00 pm.

• See an average of 15 patients per day
• Longer authorized appointments (45 minutes)

Required Qualifications

• Doctor of Medicine (MD) or Osteopathic Medicine (DO) Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of Florida or able to obtain one.
• Current, unrestricted DEA license
• Screened for TB
• Minimum of two years of experience 1+ years of experience managing a Medicare Advantage patients panel
• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency.
• Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

• Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
• The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
• $255,000 - $298,500 per year
• This job is eligible for a bonus incentive plan.
• This incentive opportunity is based upon company and/or individual performance.
