Job summary

Conviva Senior Primary Care is seeking a primary care physician for our Jacksonville, FL area clinic. The role reports to the Associate Medical Director and is onsite at the Callahan, FL clinic.

Responsibilities include seeing 12-15 patients daily with longer 45-minute visits, Monday–Friday, 8:00am–5:00pm, on-site in Callahan. Ideal candidates have MD/DO, board certification/eligibility in geriatrics, family, or internal medicine, plus state license and DEA.

Qualifications

• MD/DO board-certified or eligible in geriatrics, family medicine, or internal medicine.
• Active, unrestricted license in the state of employment.
• Current, unrestricted DEA license.
• TB screening completed.
• Minimum of two years of value-based care experience.
• 1+ years of experience managing Medicare Advantage panel.

Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Jacksonville, FL. As a Primary Care Physician, you will report to the Associate Medical Director. You will be onsite at the Callahan, FL. clinic.

A day in the life of a Conviva physician

• Monday-Friday 8:00am-5:00pm


• See 12-15 patients per day


• Longer authorized appointments (45 minutes)


• You will be on-site

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine


• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine


• Valid, unrestricted license in the state of desired employment (or willing to obtain)


• Current, unrestricted DEA license


• Screened for TB


• Minimum of two+ years of experience in value base care


• 1+ years of experience managing Medicare Advantage panel of patients


• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government. #LI-FM1

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
