• NP Board Certification with primary care, family medicine, or internal medicine background.
• Current, valid and unrestricted ARNP license and valid DEA number.
• English/Spanish bilingual proficiency is essential.
• Minimum 3-5 years of primary care experience.
• Minimum 2+ years with casemanagement emphasizing health prevention and population health management.
• Documentation of proof of a negative TB test in the last 12 months before start-date.
• Team-based care experience.
• Experience working in EMR systems.
• Experience with Google Workspace.
