Glia Health Management in Chula Vista, CA seeks an advanced practice provider (NP/PA) to join a neurology-focused outpatient team. You will deliver high-quality patient care, work collaboratively with neurologists, and support patients with complex neurological conditions.

Ideal candidates are bilingual in English and Spanish and have strong communication and leadership skills. DEA and CA license are required, with neurology experience preferred.

Responsibilities

• In collaborative practice with the clinic neurologists and NPs, the Nurse Practitioner/ Physician Assistance is responsible for providing independent clinical assessment and patient care management services for Neurological patients.
• The NP/PA must be self-directed and function with minimal direct supervision. The NP/PA functions in an interdependent role by implementing the appropriate Standardized Procedures (SPs).
• The NP/PA will provide and coordinate the care of patients, provide patient/family education, promote safe continuity of care. The NP also participates in clinical meetings, provides education and support, as well as makes appropriate referrals.
• Document services rendered.
