Qualifications
- NP Board Certification with primary care, family medicine, or internal medicine background.
- Current, valid and unrestricted ARNP license and valid DEA number.
- English/Spanish bilingual proficiency is essential.
- Minimum 3-5 years of primary care experience.
- Minimum 2+ years with casemanagement emphasizing health prevention and population health management.
- Documentation of proof of a negative TB test in the last 12 months before start-date.
- Team-based care experience.
- Experience working in EMR systems.
- Experience with Google Workspace.
