Glia Health Management in California seeks an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our neurology outpatient clinic in Chula Vista. You will deliver independent clinical assessments and manage patient care in collaboration with clinic neurologists and NPs.

Qualifications

• DEA
• Previous experience in neurology preferred

Responsibilities

• In collaborative practice with the clinic neurologists and NPs, the Nurse Practitioner/ Physician Assistance is responsible for providing independent clinical assessment and patient care management services for Neurological patients.
• The NP/PA must be self-directed and function with minimal direct supervision. The NP/PA functions in an interdependent role by implementing the appropriate Standardized Procedures (SPs).
• The NP/PA will provide and coordinate the care of patients, provide patient/family education, promote safe continuity of care. The NP also participates in clinical meetings, provides education and support, as well as makes appropriate referrals.
• Document services rendered.
