Upperline Health Inc is seeking a Primary Care Medical Assistant to provide high-quality, patient-centered care in a value-based practice. The role supports in-clinic duties, care coordination, and patient outreach in a team-based setting.

The ideal candidate is organized, patient-focused, and comfortable working at the top of their scope. National City, CA office with travel to La Mesa and Chula Vista facilities is included.

Qualifications

• Certified Medical Assistant or minimum 2 years of experience in a medical primary care setting.
• Experience in a primary care / outpatient clinic setting strongly preferred.
• Phlebotomy certification strongly preferred.
• Knowledge of medical terminology, disease processes, and basic pharmacology.
• Ability to interpret provider documentation, diagnostic orders and treatment plans.
• Clinical care coordination or care management experience is ideal.

Responsibilities

• Room patients, obtain and document vital signs, medical history, and chief complaints.
• Assist NP with health assessments and in-clinic visits for Upperline Plus patients.
• Facilitate and support virtual provider encounters from the clinic setting.
• Document clinical information in the electronic health record (EHR).
• Conduct medication reconciliation and resolve discrepancies.
• Coordinate ongoing preventative care and outreach to patients.
