Job summary

Upperline Health Inc. is seeking a detail-oriented Primary Care Medical Assistant to join a value-based care team. The role focuses on rooming patients, collecting histories, vital signs, and ensuring follow-up through outreach and care coordination.

You will support the Primary Care Provider to maintain efficient clinic operations and collaborate with an interdisciplinary team in a patient-centered setting. Strong organizational and teamwork skills are essential.

Qualifications

• Certified Medical Assistant or minimum of two years’ experience in primary care.
• MA certification and/or phlebotomy certification preferred.
• EMR experience, AthenaHealth ideal.
• Proficiency in Microsoft Office tools.
• Reliable transportation and valid driver’s license.
• Excellent customer service and communication skills.
• Ability to multitask in a fast-paced environment.

Responsibilities

• Room patients; obtain and document vital signs, medical history, and chief complaints.
• Assist providers with in-clinic visits and health assessments.
• Support virtual provider encounters from the clinic setting.
• Document clinical data in the EHR.
• Perform medication reconciliation and identify discrepancies.
• Conduct telephonic outreach to coordinate preventive and ongoing care.
• Manage in-basket messages, prescription refills, and prior authorizations.
• Support referrals, scheduling, records, and DME coordination.
• Assist patients with accessing community resources.
• Prepare and maintain clinical equipment and supplies.
• Collaborate with interdisciplinary team members.
• Participate in team huddles and meetings.
• Perform injections, phlebotomy, POCT.
